Upperline Health is seeking a Nurse Practitioner to provide clinic-based care in our Metro Atlanta market at the Austell Clinic. In collaboration with the local physician team, the APP develops complex care plans, delivers diagnostics and treatments, and educates patients and families on medications, diet, and prevention in clinic, home, or via telehealth.
Ideal candidates have primary care/internal medicine experience or value-based care exposure.
