Senior Nurse Practitioner – Transitional Care

What You'll Do
- Provide Exceptional Patient Care: Deliver comprehensive, patient-centered care in collaboration with physicians and interdisciplinary teams.
- Coordinate seamless transitions of care from admission through discharge.
- See patients in a Coordinated Care Center, providing steppingstones to successful transition to the ambulatory setting.
- Educate patients and families on disease management, medications, prevention, and healthy lifestyles.
- Advocate for patients by addressing medical, social, and behavioral health needs while coordinating specialty referrals and community resources.
- Facilitate timely, safe discharges that improve patient experience and reduce readmissions.
Drive Quality & Innovation
- Partner with multidisciplinary teams to improve patient safety, length of stay, cost of care, and patient satisfaction.
- Help shape organizational policies and innovative models of care that improve healthcare delivery.
Required Qualifications
What We're Looking For
- Master of Science in Nursing with completion of an accredited Nurse Practitioner program.
- Current Maryland Nurse Practitioner license and national certification in specialty area.
- DEA and CDS eligibility.
- ACLS/BLS certification (as applicable).
- Minimum of three years of Nurse Practitioner experience.

Work Experience
