To deliver comprehensive patient care remotely, the full-time Multi-State Licensed Nurse Practitioner will provide holistic, evidence-based assessments, diagnoses, treatments, and health counseling for patients aged 18 months and above while collaborating with a dedicated healthcare team. Key responsibilities Provide holistic, evidence-based care including assessment, diagnosis, treatment, and health counseling for patients Evaluate and manage primary care, acute, chronically ill, and transitional care patients, including health education and counseling Document patient care within an EHR and coordinate care with internal and external colleagues to ensure high standards of care Required qualifications Current unrestricted licensure in FL, OH, MA, RI, DE, MD, or Washington D.C Completion of a Master's Degree level Family Nurse Practitioner program with National Board Certification Minimum of two years of medically relevant experience or equivalent Ability to work autonomously and collaboratively within a healthcare team Proficiency in information management and technology; bilingual skills may be required depending on the market
