To support a growing telehealth initiative, the full-time State Licensed Virtual Care Nurse Practitioner will provide holistic, evidence-based patient care remotely, working weekends with a schedule from 2:30 p.m. to 11:00 p.m. while collaborating with a dedicated healthcare team. Key responsibilities: Deliver holistic care through assessment, diagnosis, treatment, and management of health problems for patients aged 18 months and above Provide patient counseling on topics such as pregnancy prevention, STI prevention, and contraceptive care Document patient care accurately within an EHR and coordinate care with internal and external colleagues to ensure high-quality service Required qualifications: Current licensure in CT, GA, NC, ND, NJ, NY, PA, SC, SD, VA, WI, or WV Minimum of two years of medically-relevant experience or equivalent Completion of a Master's Degree level Family Nurse Practitioner program with National Board Certification Ability to work autonomously and effectively within a patient care team Proficiency in information management and technology
