We are seeking a compassionate and motivated Nurse Practitioner to provide high-quality mobile and community-based healthcare services.
What You'll Do
- Diagnose and treat acute and chronic conditions
- Provide care in homes, Assisted Living Facilities (ALF), and Skilled Nursing Facilities (SNF)
- Conduct patient assessments, develop treatment plans, and prescribe medications
- Utilize telehealth and in-person visits
- Collaborate with patients, families, and healthcare partners
- Maintain accurate and timely clinical documentation
Qualifications- Master's Degree from an accredited NP program
- Current state licensure and prescribing authority
- Valid driver's license and reliable transportation
- Strong communication and clinical decision-making skills
- Knowledge of HIPAA and healthcare regulations
