HarmonyCares is seeking a Nurse Practitioner to deliver home-based primary care to medically complex, home-limited adults in the Cleveland area. In this role, you will conduct comprehensive in-home assessments, diagnose and manage acute and chronic conditions, prescribe medications, and coordinate care with an interdisciplinary team. You’ll combine in-person visits with telehealth follow-up to improve outcomes and reduce avoidable hospitalizations.
