In this physician-led, interdisciplinary role, responsibilities include performing comprehensive in-home assessments , managing chronic and acute conditions , and coordinating care through in-person visits and telehealth.
You will collaborate closely with physicians, nurses, and social workers to improve health outcomes, increase access, and reduce avoidable hospitalizations for vulnerable patients.
Qualifications and skills include a valid Nurse Practitioner license, strong assessment and care management skills, and the ability to work effectively within a team.
