About the Role
We’re seeking a Certified Registered Nurse Practitioner (CRNP) to provide compassionate, high-quality primary care to patients across the lifespan—from infants to older adults. This role focuses on preventive care, chronic disease management, and treating acute conditions in a collaborative, team-based environment.
What You’ll Do
- Provide primary care using evidence-based guidelines
- Perform exams, diagnose conditions, and manage treatment plans
- Deliver preventive care, immunizations, and patient education
- Manage chronic and acute health conditions
- Follow up on labs, imaging, and specialty referrals
- Collaborate with physicians and care teams
- Participate in case reviews, quality initiatives, and community outreach
- Maintain accurate, timely documentation and ensure HIPAA compliance
Schedule & Expectations
- Full-time role with at least one evening per week
- Shared on-call coverage (weekends/holidays)
- Team-oriented, mission-driven work environment
What You Bring
- Master’s degree in Advanced Practice Nursing
- Current or eligible PA CRNP and RN licenses
- Prescriptive authority eligibility in Pennsylvania
- CPR certification
- Eligible for insurance credentialing (Medicare/Medicaid/commercial)
- Experience in primary care or community health (preferred)
- Bilingual English/Spanish preferred
