About the Role

We’re seeking a Certified Registered Nurse Practitioner (CRNP) to provide compassionate, high-quality primary care to patients across the lifespan—from infants to older adults. This role focuses on preventive care, chronic disease management, and treating acute conditions in a collaborative, team-based environment.

What You’ll Do

  • Provide primary care using evidence-based guidelines
  • Perform exams, diagnose conditions, and manage treatment plans
  • Deliver preventive care, immunizations, and patient education
  • Manage chronic and acute health conditions
  • Follow up on labs, imaging, and specialty referrals
  • Collaborate with physicians and care teams
  • Participate in case reviews, quality initiatives, and community outreach
  • Maintain accurate, timely documentation and ensure HIPAA compliance

Schedule & Expectations

  • Full-time role with at least one evening per week
  • Shared on-call coverage (weekends/holidays)
  • Team-oriented, mission-driven work environment

What You Bring

  • Master’s degree in Advanced Practice Nursing
  • Current or eligible PA CRNP and RN licenses
  • Prescriptive authority eligibility in Pennsylvania
  • CPR certification
  • Eligible for insurance credentialing (Medicare/Medicaid/commercial)
  • Experience in primary care or community health (preferred)
  • Bilingual English/Spanish preferred
