In this role, you’ll conduct in-home and telehealth visits, perform comprehensive assessments, and manage chronic and acute conditions in collaboration with a physician-led, interdisciplinary team. You’ll coordinate care to reduce avoidable hospitalizations, educate patients and caregivers, and document in the EHR.
