In this role, you will perform comprehensive assessments, manage chronic conditions, and develop individualized care plans that prioritize patient goals and quality of life. You will conduct in-home visits and telehealth follow-ups, order and interpret diagnostics, prescribe medications, and collaborate closely with an interdisciplinary team including physicians, nurses, social workers, and care managers.
