Position Overview
Primary Care Nurse Practitioner with 1+ years of nursing home or geriatric experience needed to provide primary care services at a nursing home facility in Pittsburgh, PA.
Compensation $11,500 annually
Schedule Flexible scheduling! 8-10 hours per month
Location Nursing home in Pittsburgh, PA
Key Responsibilities
- Monthly CCM follow‑ups — Provide structured, chronic care check‑ins for eligible residents.
- Care plan creation & updates — Develop and maintain comprehensive, patient‑centered care plans for chronic conditions.
- Medication reconciliation — Review medications monthly to ensure safety, accuracy, and adherence.
- Coordination with specialists — Communicate with cardiology, psychiatry, physiatry, and other specialists involved in chronic disease management.
- Monitoring chronic conditions — Track symptoms, labs, vitals, and disease progression for residents with two or more chronic illnesses.
- Documentation for CMS billing — Record time spent, care coordination activities, and care plan updates to meet CCM billing standards.
Required Qualifications
- Active Nurse Practitioner license in Pennsylvania with Board Certification.
- 1+ year of nursing home or long‑term care experience.
- Strong clinical judgment and comfort working independently.
