Total Extended Care Services, a growing Physical Medicine and Rehab group is looking for a dedicated Nurse Practitioner/Physician Assistant to join our team. In this role, you will partner with our PM&R physician in the sub‑acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.

Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care.
- Communication and collaboration with the patient's primary care team to ensure the best outcomes.
- Timely charting/documentation is required but can be done from home.
