Total Extended Care Services, a growing Physical Medicine and Rehab group is looking for a dedicated Nurse Practitioner/Physician Assistant to join our team. In this role, you will partner with our PM&R physician in the sub-acute rehab setting. This position is perfect for those passionate about improving patient outcomes and working within an interdisciplinary, specialized team.
About the Role
Location: In‑person at skilled nursing facilities.
Schedule: Flexible options, Full‑time and Part‑time positions available.
Culture: Detail‑oriented, outcome‑focused, stable, people‑oriented, and team‑oriented.
Responsibilities
- Partner with the facility's therapy department to provide the best possible rehab care.
- Communicate and collaborate with the patient's primary care team to ensure the best outcomes.
- Perform timely charting/documentation, which can be done from home.
