About the Role

Our Nurse Practitioner will perform comprehensive health assessment which include but not limited to the following:

  • Obtain history and perform physical examination
  • Complete review of medicines and related instructions
  • Review home safety and psychosocial needs
  • Fulfill documentation needs for STAR and HCC measures
  • Obtain and document all pertinent information in patient's charts and case management notes on same day as patient is seen; include medications and end of life preferences and "drivers" of preventable hospital admissions
  • Instruct all patients, caregivers and family to call you prior to accessing medical care, subject to life threatening emergencies
  • Maintain patient safety and confidentiality Universal precautions utilized in all patient care
  • Prescribe/order procedures, treatments and medications within scope of practice
  • Develop high level of trust and intimacy with patients and families

Skills, Experience & Education You Bring

  • CA Licensed Nurse Practitioner
  • 1+ Years of Experience with Primary Care

Salary and Benefits

The Base Salary for this position is $135,000 - $155,000/year + Sign on Bonus! In addition, we provide a competitive compensation package that recognizes your experience, credentials, and education alongside a robust benefits program to meet your needs.

The pay scale listed for this position is generally for candidates that meet the specified qualifications and requirements listed on this job description.

Additional pay may be determined for those candidates that exceed these specified qualifications and requirements.

Physical Requirements

Office Environment - roles involving part to full time schedule in Office Environment. Based in our physical offices and work from home office/deskwork – Activity level: Sedentary, frequency most of workday.

External hires must pass a background check/drug screen.
