• Provide direct patient care, emphasizing health promotion, illness prevention, and continuity of care.
• Conduct comprehensive histories, physical exams, and order appropriate tests.

• Participate in quality improvement, team collaboration, and support the Patient Centered Medical Home model.
• Supervise students and staff, contribute to community health initiatives, and maintain compliance with regulations.
• Qualifications include NY licensure as a Nurse Practitioner or Physician Assistant, with experience in primary care. Bilingual abilities and community health experience are preferred.

Salary ranges from $100,000 to $140,000 annually. All qualified applicants are encouraged to apply regardless of background.
