Responsibilities
- Deliver primary and specialty care to patients across Alianza's Puerto Rico network
- Conduct comprehensive patient assessments, diagnoses, and treatment planning within your scope of practice
- Manage chronic conditions and close preventive care gaps for an underserved patient population
- Order and interpret diagnostic studies, prescribe medications, and coordinate referrals
- Collaborate with supervising physicians, care managers, and behavioral health specialists on integrated care plans
- Document patient encounters and care activities in the MyChron platform
- Build long-term, culturally attuned relationships with patients and their families
Requirements
- Active or eligible Puerto Rico NP or PA licensure (Alianza supports credentialing and licensure transfer)
- Graduate of an accredited Nurse Practitioner or Physician Assistant program
- Current national certification (AANP, ANCC, or NCCPA)
- Bilingual English/Spanish required; strong Spanish fluency essential for patient communication
- 2+ years of clinical experience preferred; new graduates with strong bilingual skills considered
