Physical Medicine and Rehabilitation (PM&R), also known as Physiatry, focuses on restoring functional ability and quality of life for patients with physical impairments. As a Nurse Practitioner or Physician Assistant PM&R Clinician with Comprehensive Rehab Consultants (CRC), you will provide specialized services exclusively at a designated skilled nursing facility (SNF) or post‑acute rehab center in Glendale, AZ.

Responsibilities

  • Provide specialized PM&R / Physiatry services as a rehab consultant.
  • Represent CRC in the facility through strong clinical leadership and collaboration with the interdisciplinary care team.
  • Collaborate with primary care teams to manage conditions common in SNFs such as cerebrovascular accidents, progressive neurological conditions, and postoperative orthopedic surgeries.
  • Work with PT, OT, and SLP teams to improve patient function and quality of life.
  • Follow therapy patients to track their progress.
  • Assess and collaborate on management of barriers to therapy—including joint or neuropathic pain, spasticity, neurogenic bowel, neurogenic bladder, and others.
  • Participate in multidisciplinary meetings, bridging the gap between therapy and internal medicine.
  • Engage in culture building opportunities and ongoing education.

Qualifications

  • Nurse Practitioner, APRN, or Physician Assistant.
  • Arizona license.
  • Ability to work in the United States.
  • Experience in post‑acute space preferred.
  • Positive attitude and commitment to delivering the best patient care.
  • Availability to work 40 hours per week.

Position Details

  • Location: Glendale, AZ.
  • Employment type: Full‑time, 40‑hour week, W‑2.
  • Shift: 8‑hour shift, 9A.M.–4P.M., Monday–Friday with flexible days.
  • Work ethic: no evenings, weekends or on‑call obligations.
  • Ability to commute or relocate to Glendale, AZ before start.
  • License required: Nurse Practitioner/APRN/PA.

Compensation & Benefits
