POSITION DESCRIPTION

The Nurse Practitioner or Physician Assistant will deliver comprehensive primary care, chronic disease management, and preventive health services to a diverse patient population via in-home visits (house calls) in a designated territory in the New York metro area. The exact geographic boundaries for the position will be mutually determined during the interview process.

  • in-home assessments, physical exams, medical histories, and diagnostic evaluations
  • health screenings and wellness checks
  • preventive care, health education, and counseling
  • develop and implement individualized care plans
  • work in collaboration with patients, families, and interdisciplinary healthcare teams
  • manage chronic conditions (diabetes, hypertension, heart disease, etc.)
  • order, interpret, and follow up on diagnostic tests, labs, and imaging
  • prescribe medications and treatments in accordance with scope of practice
  • administer immunizations
  • address urgent medical needs in the home when possible
  • provide timely follow-up care for patients discharged from hospitals, EDs, or SNFs
  • regularly evaluate patients to prevent readmissions
  • ensure continuity of care with primary care providers, specialists, and community resources
  • participate in quality improvement initiatives
  • provide culturally-sensitive and linguistically-appropriate care at all times

The practice uses the MDLand EHR.

The provider will work with a majority Chinese-speaking patient population and must be fluent in Mandarin and/or Cantonese.

REQUIREMENTS

  • New York NP or PA license
  • DEA
  • relevant board certification (PA-C, FNP, AGNP, etc.)
  • 1+ year advanced practice experience in a relevant setting
  • experience with home or community-based care strongly preferred
  • must speak Mandarin or Cantonese fluently
