We are seeking a motivated and skilled mid-level provider to join our outpatient endocrinology team serving South Riverside County. In this role, you will take a lead role in the assessment, treatment, and ongoing management of patients with endocrine and metabolic disorders — including diabetes, thyroid disease, adrenal conditions, and other hormonal pathologies.

You will work within a multidisciplinary team environment, rounding with a degree of clinical autonomy while collaborating closely with physicians, dietitians, and nursing staff to ensure comprehensive, coordinated patient care. Candidates with prior acute or hospital-based experience are strongly encouraged to apply.

Key Responsibilities

• Clinical Assessment: Perform comprehensive endocrine evaluations including detailed history, physical examination, laboratory interpretation, and risk stratification for diabetes and hormonal disorders.
• Treatment & Disease Management: Develop individualized, evidence-based treatment plans for conditions including diabetes mellitus (Type 1 and Type 2), thyroid disorders, osteoporosis, adrenal and pituitary conditions, and metabolic syndrome.
• Patient Education: Empower patients and their families through education on disease processes, lifestyle modifications, medication adherence, and self-management strategies.
• Collaborative Care: Partner with physicians, dietitians, nurses, and other specialists to coordinate care, optimize outcomes, and support a team-first culture.
• Procedures & Documentation: Perform and interpret point-of-care testing (glucose monitoring, A1C, thyroid ultrasound if trained); adjust medications accordingly. Maintain thorough, accurate clinical documentation including assessments, treatment plans, progress notes, and outcomes.
• Quality & Advocacy: Participate in quality improvement initiatives and advocate fiercely for each patient's access to comprehensive, compassionate care.

Qualifications

Education & Certification

• Master's degree in Nursing with Nurse Practitioner certification required.
• Board certification as a Family Nurse Practitioner (FNP) or Adult-Gerontology Nurse Practitioner (AGNP) preferred.
• Physician Assistant candidates with equivalent credentials will be considered.

Licensure

• Current California NP or PA license.
• Active DEA registration.
• APRN certification (for NP candidates).

Experience

• Minimum 1 year of clinical experience as a Nurse Practitioner or Physician Assistant.
• Prior exposure to endocrinology, diabetes management, or internal medicine strongly preferred.
• At least 1 year of acute or hospital-based experience preferred.
