You will work onsite at our Southside clinic in Corpus Christi, Texas. You will report to our Associate Medical Director.

Your Role & Responsibilities
- You will provide comprehensive care to seniors following clinical standards.
- You will collaborate daily with your care team to ensure coordinated, high-quality care.
- You will manage referrals, hospital/SNF coordination, DME, and home health services.
- You will document care accurately and efficiently with support from quality-based coders.
- You will participate in on-call rotation and support growth plans at your center.
- You will have passion for providing value-based care.
What We're Looking For
- You will have a Master's Degree in Nursing or completion of a PA program with board certification.
- You will have an active, unrestricted NP or PA license in the state of practice.
- If selected for this role, you will be required to be screened for TB.
Preferred
- You will have 2 or more years of experience in value-based care, primary care or Medicare Advantage settings.
- You will have an active DEA license and Medicare/Medicaid provider numbers or the ability to obtain DEA license and/or Medicaid and Medicare numbers.

Pay Range
$123,800 - $170,300 per year. This job is eligible for a bonus incentive plan based on company and/or individual performance.
