In-Home Primary Care Provider (NP/PA)
Position Type: Full-time
Location: Lake / Orange County, Florida
Overview:
We are currently seeking a dedicated, compassionate, and experienced Primary Care Provider (Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant) to deliver comprehensive healthcare services to patients in their homes or in community-based settings such as assisted living facilities, skilled nursing facilities, or group homes. This role offers a meaningful opportunity to improve patient outcomes and promote continuity of care through personalized, relationship-based visits.
Key Responsibilities:
- Conduct comprehensive in-home assessments, including medical history reviews, physical exams, and medication management — minimum of 25 visits per week within an assigned territory.
- Diagnose and develop individualized, evidence-based treatment plans.
- Manage chronic health conditions such as diabetes, hypertension, and cardiovascular disease.
- Prescribe, adjust, and manage medications while educating patients on proper use and potential side effects.
- Provide preventive care including immunizations, screenings, and wellness counseling.
- Educate patients on disease management and healthy lifestyle practices.
- Serve as the patient’s primary care provider, coordinating with other healthcare professionals to ensure seamless, integrated care.
- Maintain timely and accurate documentation within an EMR system, completing notes within 48 hours of visits.
- Demonstrate exceptional bedside manner and foster trust-based patient relationships.
- Participate in regular follow-ups and quality assurance initiatives.
- Be available for after-hours, on-call duties as needed.
- Travel within assigned territory; reliable transportation required.
Required:
- Active, unrestricted NP or PA license in the state of practice.
- National certification as a Family Nurse Practitioner, Adult-Gerontology Nurse Practitioner, or Physician Assistant.
- DEA license.
- Current Basic Life Support (BLS) certification for healthcare providers.
- Valid driver’s license (minimum 3 years) with a clean driving record and reliable transportation.
- Proficiency in EMR systems and strong computer literacy.
- Strong clinical assessment and diagnostic skills.
- Ability to pass comprehensive background and drug screening.
Preferred:
- 1-2 years of experience in primary care, home-based care, family practice, or outpatient care.
- Experience with HEDIS/STAR measures.
- Familiarity with Hierarchical Condition Category (HCC) documentation.
- Previous home visit-based care experience.
Physical & Working Conditions:
- Ability to lift up to 20 lbs.
- Ability to sit, stand, bend, kneel, and navigate various residential environments.
- Comfort working independently in patient homes, including environments with varying conditions.
- Mental acuity to assess, diagnose, document, and problem-solve patient needs.
- Self-motivated and mission-driven, with a genuine passion for patient-centered care.
- Skilled in relationship-building and providing empathetic, culturally competent care.
- Proficient in managing multiple priorities with excellent organizational and time management skills.
- Strong understanding of CMS and ACA risk adjustment models (preferred).
1040 probationary period with Medix, then conversion to client company.
