Overview
We are seeking experienced and motivated Nurse Practitioners to perform in‑home Health Risk Assessments (HRAs) for patients throughout Snohomish County, WA. This is an excellent opportunity for providers looking to supplement their income with a flexible, part‑time schedule.
Key Responsibilities
- Conduct comprehensive in‑home Health Risk Assessments (HRAs)
- Identify and document acute and chronic medical conditions
- Obtain vital signs and calculate BMI
- Review medications and pharmacological therapy
- Perform a physical examination
- Evaluate patients’ physical, mental, and social health needs
- Educate and counsel patients and their families on health conditions and preventative care
Schedule & Flexibility
- Part‑time, flexible scheduling
- Set your own hours
- Visits available 7 days a week
- Minimum commitment of 8–10 hours per week
Compensation
- $110 per completed in‑home assessment
- Mileage reimbursement provided
- No‑show compensation included
Qualifications
- Active Nurse Practitioner license in the state of Washington
- Experience with Health Risk Assessments (HRA) preferred
- Strong clinical assessment and documentation skills
- Ability to work independently and manage schedule efficiently
- Reliable transportation for in‑home visits
