Nurse Practitioner, Palliative Care - Atlantic Health Chilton Medical Center

Job Description

Responsible for working alongside multiple internal/external physician and health care professionals and facilitating advanced primary and specialty nursing services. Provides general and preventative care for adults at the end-of-life, treat illnesses, discuss goals of care, order lab tests, and prescribe medication in the inpatient and outpatient/ambulatory setting.

Principal Accountabilities

• Provides new patient consultation and reviews health history.
• Performs physical examinations and screening on patients.
• Provides after-care/ongoing care information and resources to patients and their families.
• Ensures patient records are maintained including test results, immunizations, condition changes.
• Delivers quality, advanced practice care while complying with company policies, safety regulations, and productivity metrics.
• Recommends further treatment options or future appointments if required.
• Performs diagnosis of health problems and provides medications.
• Performs other related duties as assigned.

Qualifications

• Master’s degree in Nursing in area of specialty practice; or post‑master’s certificate in area of specialty practice with graduate degree in nursing
• Current state APN certificate
• Current RN license

Preferred

• 5 or more years experience in Nursing or relevant area.
