Optum's HouseCalls program is seeking Nurse Practitioners and Physician Assistants to provide annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, identifying health risks, closing care gaps, and helping patients stay healthy in their homes.
What You'll Do
- Conduct comprehensive in-home health assessments
- Review medical history, medications, and vital signs
- Perform physical exams and evidence-based screenings
- Identify and document diagnoses for care planning
- Educate patients on conditions, medications, and treatment compliance
- Address social determinants of health and connect patients with resources
- Communicate findings and recommendations to primary care providers
- Recognize and respond to urgent or emergent health concerns

Ideal Candidate
- Certified NP (AANP/ANCC) or PA (NCCPA)
- Active state licensure (or ability to obtain)
- Comfortable working independently in patients' homes
- Strong communication and assessment skills
- Experience with geriatric, family practice, home health, or Medicare populations preferred
