About the Role Optum HouseCalls is seeking Nurse Practitioners and Physician Assistants to conduct annual in-home health assessments for Medicare Advantage and other health plan members. This is a non-prescribing, field-based role focused on preventive care, patient education, identifying care gaps, and collaborating with primary care providers.
Key Responsibilities
- Conduct comprehensive in-home health assessments
- Review medical history and medications
- Perform physical exams, screenings, and point-of-care testing
- Identify and document diagnoses for treatment planning and care management
- Communicate findings to primary care providers
- Educate members on disease management and medication compliance
- Address social determinants of health and connect members with resources
- Recognize and respond appropriately to urgent or emergent situations
Required Qualifications
- NP: ANCC or AANP certification in Family, Adult, Geriatric, or Adult-Gerontology Primary Care
- PA: NCCPA certification and active state licensure
- Active, unrestricted license (or ability to obtain by start date)
- Valid driver's license and reliable transportation
- Ability to work independently in various home environments
Preferred Qualifications
- 1+ years of clinical experience in family practice, geriatrics, primary care, or home health
- Experience working with Medicare or geriatric populations
- EMR and technology proficiency
- Strong communication and patient education skills
- Ability to transport equipment up to 30 pounds and navigate stairs during home visits
