Role Overview
Seeking a provider to conduct comprehensive in-home health assessments for adult and geriatric members, lasting up to one hour. Responsibilities include reviewing medical history, medication adherence, social factors, administering tests, collaborating with healthcare professionals, and documenting findings. Services are provided at members' residences, including homes and Skilled Nursing Facilities.
Key Responsibilities
- Perform assessments, including medical history, diagnosis, treatment, and health education
- Order and administer point-of-care and lab tests
- Coordinate with physicians, case managers, social workers, and caregivers
- Follow up with primary care physicians and provide patient education
- Work primarily in members' residences
Qualifications & Skills
- Master’s degree or equivalent experience with NP or PA licensure
- Current licensure with prescriptive authority, certification in relevant specialties
- Valid CPR/BLS/ACLS certifications
- Minimum of one year NP or PA experience
- Ability to travel extensively, including overnight stays
- Valid driver’s license and insurance; strong computer and communication skills
