OnPoint Medical Group is seeking a Nurse Practitioner or Physician Assistant to provide high-quality primary care within a collaborative, physician-led team. You will perform comprehensive assessments, manage acute and chronic conditions, order and interpret diagnostic tests, prescribe medications, and coordinate care across specialties. The role emphasizes patient-centered communication, preventive care, and health education for individuals and families. You'll work closely with physicians, behavioral health, and ancillary services to ensure seamless, coordinated care and strong patient relationships throughout the Denver metro community.
Responsibilities
- Provide comprehensive primary care evaluations and ongoing management for patients across the lifespan
- Diagnose and treat acute and chronic conditions following evidence-based guidelines
- Order, interpret, and act on laboratory tests, imaging, and other diagnostics
- Prescribe medications and manage treatment plans in collaboration with supervising physicians
- Deliver preventive care, wellness visits, and patient education on disease prevention and lifestyle
- Coordinate care with physicians, behavioral health, and ancillary services to support patient outcomes
- Maintain accurate, timely documentation in the EMR system
- Participate in quality improvement initiatives and adhere to clinical and organizational protocols
- Build strong, respectful relationships with patients, families, and the care team
