Primary care Nurse Practitioner in a home-based setting with focus on geriatric chronic condition management. Work Monday through Friday, no weekends or evenings. Spend 30-45 minutes with patients, chart during the workday with Athena and AI Scribe.

Responsibilities

The Nurse Practitioners we seek have direct experience in home health, geriatrics, hospice, and primary care. They focus on patient care rather than volume, taking the time needed to address patient needs.

Qualifications

• Licensed as a NP in the state where HarmonyCares Medical Group office located
• Federal DEA License/eligibility
• Geriatric training/experience preferred
• Active CPR/BLS Certification
• Board Certification
• Valid driver’s license and good driving record
