Key Responsibilities:
- Provide high-quality medical care to residents in a SNF setting, focusing on reducing unnecessary hospitalizations and promoting effective treatment in place
- Conduct thorough assessments, develop individualized care plans, and order necessary treatments and medications
- Collaborate with facility staff, including physicians, nurses, and other healthcare professionals to ensure seamless continuity of care
- Communicate effectively with families, residents, and healthcare providers to ensure open lines of communication
- Stay up-to-date with industry best practices, regulations, and survey processes to ensure compliance and continuous improvement
Requirements:
- Salary: $105,000 - $152,000 ANNUALLY
