nYou’ll step into an outpatient primary care role focused on seniors with complex chronic conditions. You’ll have your own care team and strong leadership support while practicing within a value-based care model.

n• Monday–Friday, 8:00 AM–5:00 PM
• Approximately 10–12 patients per day
• Maximum panel of 650 patients
• Call frequency based on the provider team size
• Dedicated MA, scheduler, and referral specialist

What makes you a great fit
n• You are a Nurse Practitioner with 2+ years of outpatient primary care experience
• You’re comfortable caring for seniors with multiple complex conditions
• You can perform suturing, joint injections, skin biopsies, and basic wound care
• You hold an active, unrestricted license and current DEA registration
• You value patient-centered, team-based care
• Geriatric experience is a plus, but not required
Compensation & Benefits
n• Base salary: $130,000
• Up to $13,000 annual bonus
