Your Responsibilities

• Performing thorough patient evaluations, ordering and interpreting necessary diagnostic tests, and devising tailored care plans
• Diagnosing and managing acute, episodic, or chronic ailments in conjunction with physicians or autonomously (according to your credentials)
• Instructing patients and their families about preventative measures, wellness strategies, and disease management techniques
• Prescribing medications and therapies as permitted by your practice scope
• Engaging in patient follow-up and overseeing chronic care management
• Collaborating with an interdisciplinary team to provide integrated, patient-centered care
• Keenly documenting patient interactions and care plans within electronic medical records (EMR)

Qualifications We Are Looking For

• Master of Science in Nursing (MSN) or Doctor of Nursing Practice (DNP)
• Completion of an accredited Nurse Practitioner program

Essential Certifications

• Valid Nurse Practitioner license in the state of Kentucky
• National certification as a Family Nurse Practitioner (FNP-C, AANP or ANCC) or other specialty equivalent
• Basic Life Support (BLS) and Advanced Cardiovascular Life Support (ACLS) certifications (or ability to achieve within 90 days)
