Description of Duties:
This is a grant funded position.-Conduct geriatric assessments to evaluate older patients' functional ability, cognition and mental health,and socioenvironmental circumstances-Ensure comprehensive biannual screenings for cognition impairment, depression, activities of daily life, falls, and medication review-Develop and implement individualized care plans, in collaboration with the Care Coordinator, Social Worker, and other members of the health care team.-Provide STI screening, HCV/HBV screening, vaccinations, evaluate patients for iART and prescribe treatment.-Maintain a panel of STAR-Downstate older adults patients for primary HIV care, conducting health assessments and screenings related to HIV and aging.-Conduct biannual cardiac screening evaluation to detect the risk of developing heart disease at an early stage and refer for electrocardiogram, as indicated.-Refer for sub-specialty medical evaluations. Communicate with consultants and provide care services based on consultant's treatment recommendations-Conduct a medication list review with the clinical pharmacist to assess for polypharmacy and potential drug interactions.-Educate clients and families on aging-related issues. Assist with health care proxies, long term care planning, end-of-life planning and decision-making.-Record session notes, care coordination and patient follow-up in the EHR.-Participate in the development and implementation of multi-disciplinary case management plans, including identifying and referring patients for treatment of mental health, substance use and adherence problems.-Oversee case finding activities. Identify and track quality indicators related to care of older adults, and participate in the STAR-Downstate CQI program.-Conduct staff trainings on how to conduct a geriatric assessment.-Participate in regular STAR and SUNY administrative and educational meetings, including monthly staff meetings and provider meetings,-Promote outreach activities that would help to promote STAR-Downstate services and identify new older adult patients for care.-Assist STAR clinicians with coverage of Urgent Care (walk in} patient visits as needed and urgent linkage visits of newly diagnosed or out of care patients as needed-Assist with medication refill requests for STAR patients
Qualifications:
REQUIRED:NYS licensed nurse practitioner; 1-2 years' post-license experience; Excellent communication and interpersonal skills demonstrating patience, empathy,compassion, and strong communication skills for building trust and understanding the needs of older adults; Strong understanding of aging-related health conditions.Experience working as part of a multidisciplinary team .PREFERRED:Adult-Gerontology Nurse Practitioner Certification or experience working with older adults. Bilingual English/Haitian Creole or English/Spanish preferred.
