In this full-time role, you will conduct in-home visits and telehealth consultations, perform comprehensive assessments, manage chronic conditions, and develop individualized care plans. Working within an interdisciplinary team, you’ll coordinate services to reduce avoidable hospitalizations, support patients and caregivers, and document care in the EHR.
