Nurse Practitioner – Primary Care
The Role:
We’re seeking a Nurse Practitioner – Primary Care to provide comprehensive, patient-centered medical care across the lifespan. In collaboration with physicians and interdisciplinary care teams, you’ll assess, diagnose, treat, and manage a wide range of acute and chronic conditions while promoting health maintenance and disease prevention.
What You’ll Do:
- Perform comprehensive physical examinations and health assessments.
- Diagnose and manage acute and chronic illnesses.
- Order, interpret, and evaluate diagnostic tests, including labs and imaging.
- Develop and implement individualized treatment plans.
- Prescribe medications and therapies in accordance with state regulations.
- Provide preventive care, including immunizations and routine screenings.
- Manage chronic conditions such as diabetes, hypertension, asthma, and heart disease.
- Educate patients and families on disease management, lifestyle modifications, and preventive care.
- Perform minor office procedures such as suturing, wound care, joint injections, and skin biopsies.
- Coordinate care with specialists and other healthcare professionals.
- Document patient encounters accurately and timely in the electronic health record (EHR).
- Participate in quality improvement initiatives and population health management.
- Other relevant duties as assigned by management.
What We’re Looking For:
- Master’s or Doctorate degree in Nursing (MSN or DNP).
- Graduate of an accredited Nurse Practitioner program.
- Current Registered Nurse (RN) license.
- Current Nurse Practitioner license in the practicing state.
- National board certification (AANP or ANCC) – Family NP or Adult-Gerontology NP.
- DEA registration (if prescribing controlled substances).
- BLS/ACLS may be required depending on setting.
- Strong clinical assessment and diagnostic skills.
- Excellent communication and patient education abilities.
- Knowledge of evidence-based practice guidelines.
- Ability to work independently and collaboratively.
- Proficiency with electronic medical records (EHR/EMR).
- Cultural competency and patient-centered care focus.
- Outpatient primary care clinic.
- May include community health centers, private practices, or hospital-affiliated clinics.
