You will complete detailed histories and physicals, reconcile medications, identify care gaps, and document findings in the EMR to support value-based care. Collaborating virtually with Signify’s interdisciplinary teams, you’ll use data and clinical guidelines to guide decisions, educate patients and caregivers, and improve quality outcomes for vulnerable populations in a flexible, field-based role.
