About the Role
As a Nurse Practitioner or Physician Assistant, you'll provide longitudinal care to high-risk Medicare members in senior housing settings, including private homes, adult family homes, and assisted living communities. This role combines clinical autonomy with interdisciplinary collaboration to help improve outcomes and enhance quality of life for vulnerable patient populations.
What You'll Do
- Conduct comprehensive patient assessments, including physical exams, medication reviews, and evidence-based screenings.
- Collaborate closely with interdisciplinary care teams to coordinate patient care.
- Lead advanced illness and care-planning conversations.
- Educate patients and caregivers and ensure accurate clinical documentation.
What You'll Bring
- Active NP certification (ANCC or AANP) in Family, Adult, Geriatric, or Adult-Gerontology Primary Care, OR current NCCPA certification for PAs.
- Active, unrestricted state license (or ability to obtain prior to start).
- Active driver's license and reliable transportation.
- DEA licensure/prescriptive authority (or ability to obtain, where applicable).
Preferred Qualifications
- 1+ years of APC clinical experience, preferably in long-term care.
- Experience in geriatrics, senior living, home care, or complex patient populations.
- Familiarity with chronic illness management, advanced illness discussions, and EMR systems.
Role Details
- Location: Olympia, Washington (Lewis & Thurston County coverage)
- Schedule: No on-call responsibilities, no weekends, and no holidays required.
- Compensation: $109,500 - $164,000 total cash compensation range.
