Nurse Practitioner (NP)
We are seeking a compassionate Nurse Practitioner (NP) to join the PACE (Program of All-Inclusive Care for the Elderly) team in Grand Junction, CO. In this role, you will provide comprehensive primary care services to frail seniors, helping them remain safely in their homes and communities while collaborating with an interdisciplinary healthcare team.
Key Responsibilities
- Provide comprehensive primary care, including diagnosis and treatment of acute and chronic conditions.
- Perform preventive care, health screenings, and urgent medical evaluations.
- Conduct comprehensive health assessments in the clinic, PACE Day Center, and participants' homes.
- Develop and manage individualized treatment plans.
- Collaborate with the Medical Director and interdisciplinary care team to coordinate participant care.
- Make timely referrals to specialty providers and community resources.
- Educate patients and caregivers on disease management, medications, and preventive care.
- Maintain accurate and timely documentation in the electronic medical record.
Qualifications
- Active Nurse Practitioner (NP) license.
- Current DEA and prescriptive authority (if required).
- Experience in Primary Care, Family Practice, Geriatrics, Internal Medicine, or PACE preferred.
- Strong assessment, diagnostic, and care coordination skills.
- Ability to work independently and as part of an interdisciplinary team.
- Valid driver's license and willingness to conduct occasional in-home visits.
