Nurse Practitioner – Geriatric Primary Care (Outpatient – Full Benefits – New Grads)

An established outpatient primary care practice in Gales Ferry, Connecticut is seeking a Family Nurse Practitioner (FNP) or Adult-Gerontology NP (AGPCNP) to join a collaborative team serving southeastern Connecticut. This role offers a 4-day work week , outpatient-only setting, and strong work-life balance.

Providers deliver comprehensive primary care, including sick visits, wellness exams, chronic disease management, and some gynecologic services. New graduates are welcome , making this an excellent opportunity for early-career NPs seeking mentorship and long-term growth in a supportive practice environment.

Key Responsibilities

• Conduct comprehensive outpatient primary care visits
• Manage acute and chronic medical conditions
• Provide preventive care and health maintenance
• Deliver limited gynecologic care within scope
• Collaborate with physicians and care teams
• Maintain accurate and timely EHR documentation

Qualifications

• Strong communication and patient-centered care skills
• Outpatient or primary care experience preferred

Schedule & Compensation

• Four 10-hour shifts per week
• Outpatient-only setting
• Comprehensive benefits + CME allowance
• Salary: $130,000–$140,000 annual
