Nurse Practitioner - Geriatric Medicine

Job Description:

• Collaborate closely with PACE Organization physicians to deliver primary care services
• Perform comprehensive participant assessments (initial, ongoing, and as-needed)
• Develop, implement, and manage individualized care plans
• Provide acute illness evaluation and treatment
• Participate in discharge planning for hospital or long-term care transitions
• Conduct home visits when necessary to ensure continuity of care
• Assist in development and implementation of clinical policies and procedures
• Participate actively in interdisciplinary team (IDT) meetings
• Provide care focused on geriatric and frail populations
• Ensure accurate and timely documentation of all clinical encounters
• Participate in provider on-call rotation
• Support a compassionate, patient-centered care environment

• Board Certified NP (ANCC or AANP) - (REQUIRED)
• Active NP License - (REQUIRED) * no exceptions
• DEA Registration - (REQUIRED)
• CDS Certification - (REQUIRED)
• BLS Certification - (REQUIRED)
• Minimum 5-7 years NP experience (preferably long-term care) - (REQUIRED)
• Experience with frail/elderly populations (or willingness for training) - (REQUIRED)
• Clean Malpractice/License History - (HIGHLY PREFERRED)
• Does the provider require visa support? (J1/H1B) - (REQUIRED)
• Most recent NPDB Self Query Report (within 30 days active) - (REQUIRED) * no exemption, required at presentation
