What You’ll Do:

• Provide palliative care through evaluation and assessment of health history and diagnosing medical conditions.
• Develop individualized plans of care.
• Complete face‑to‑face assessments and prescribe medications or treatments.
• Collaborate with the interdisciplinary team.

What You Bring:

• Active NP certification (state‑licensed).
• Valid driver’s license, reliable transportation, and current auto insurance.
• Strong communication skills and a team‑oriented mindset.
• Ability to work independently in home settings.
• Commitment to patient‑focused care.
• CPR certification.

Salary: $108,000 - $125,000 annually
