- Start date: ASAP
- Mon-Fri, and once a month on a Sunday and take a weekday off 8-5pm
- EMR: EPIC
- Face to face hospice visits, working with the team and being a resource for home aids
- 45-50 visits a month.
- California NP & RN licenses
- DEA, NPI, MSN, AANP/ANCC certification, BLS
- 3+ years of hospice or palliative care experience
- ACHPN certification required within one year
