Oak Street Health is seeking a compassionate Nurse Practitioner to join our Healthcare & Medical Services team. In this role, you will provide comprehensive primary care to adult Medicare patients in underserved communities, focusing on prevention, chronic disease management, and patient education. You will conduct assessments, diagnose and treat common conditions, prescribe medications, and develop evidence-based care plans in collaboration with physicians and an interdisciplinary team.
You’ll also engage in care coordination, quality improvement initiatives, and use EMR systems to ensure safe, patient-centered, and culturally sensitive care.
