Offering a flexible PRN opportunity, the Nurse Practitioner AZ License holder will conduct telehealth visits, perform clinical assessments, and coordinate care for patients, helping them access Community Health Worker services in a virtual environment. Key responsibilities Conduct telehealth visits and perform comprehensive clinical assessments to identify health needs and social barriers to care Develop individualized care plans and provide medical clearance and referrals for Community Health Worker services Collaborate with Community Health Workers to support patient engagement and manage ongoing primary care needs Required qualifications Active unrestricted Nurse Practitioner license with prescriptive authority Licensure in at least one of the following states: AZ, CA, HI, MI, AK, LA, NM, or NY; multistate compact licensure preferred Experience in primary care, urgent care, behavioral health, or chronic disease management; telehealth experience is a plus Strong understanding of social determinants of health and their impact on patient outcomes Fluency in Spanish or other languages is strongly preferred
