Role:
Nurse Practitioner, Advanced Practice Provider
Responsibilities:
Provide effective, equitable primary care to adults on Medicare, including routine and acute visits, wellness assessments, care coordination, patient education, and participation in population health initiatives. Collaborate with a care team of physicians, nurses, and support staff, and conduct home visits as needed. Focus on building trust and long-term patient relationships within an outpatient setting.
Qualifications:
Master’s or Doctorate in Nursing, national certification in Family, Adult-Gerontology, Adult, or Gerontological Nurse Practice, active NP license, DEA license, and US work authorization. Experience in primary care and understanding of HCC, ICD-10 coding, and health risk assessments are preferred. Bilingual skills are a plus.

Strong clinical knowledge, care coordination, patient communication, teamwork, and population health management. Ability to work independently and collaboratively within a multidisciplinary team.
