CommuniCare Family of Companies seeks a Nurse Practitioner (Adult and/or Gerontology) to provide high-quality primary care in a resident-centered setting across skilled nursing, assisted living, and related communities. Telehealth may be included to enhance access and continuity of care.
The NP will diagnose and treat acute and chronic conditions, develop care plans, and collaborate with PCPs and interdisciplinary teams to reduce hospitalizations and improve patient experience.
