- Provide high-quality primary care in a high-touch, person-centered care model
- Diagnose, treat, and manage acute episodic and chronic illness
- Conduct wellness visits and detect changes early
- Provide timely treatment and health education
- Develop and communicate treatment plans to residents, providers, caregivers, and others
- Order and interpret laboratory and diagnostic testing and adjust treatment plans
- Maintain compliant documentation in the EMR software
- Collaborate with operations, clinical leadership, PCPs, interdisciplinary teams, and specialists
- Support value-based care initiatives
- Provide an excellent customer experience
- Complete required monthly, quarterly, annual, and acute visits
- Participate in family conferences and provide advanced care planning and education
- Address pharmacy recommendations and pharmacist communications
- Coordinate care with telemedicine providers
- Provide face-to-face coverage at assigned facilities during work hours
- Follow professional standards, state and federal regulations, guidelines, and laws
Requirements
- Master of Science in Nursing, Doctor of Nursing Practice, or NP Certificate Program graduate
- National certification from AANP or ANCC
- Active, unrestricted state license
- Active DEA license
- US work authorization
- 2+ years of clinical experience as a Nurse or Nurse Practitioner in a geriatric, adult, or family practice setting
- Ability to work in a flexible environment and schedule
- Passion for providing healthcare to senior and low-income populations
- Strong oral and written communication skills
- Detail-oriented and able to work independently and in a team setting
- Customer service approach with teams and residents
