APP - PART TIME - Hospital
Location: Lake County, Illinois
Schedule: Part-Time (Primary Need) | 7:00 AM - 5:00 PM | 10-Hour Shifts
Compensation: $61.00 - $79.67/hour

Position Overview
Hospital in Lake County is seeking experienced Advanced Practice Providers (PA-C or Acute Care NP) to serve in a newly created Admitting APP role .
This position is focused on identifying, evaluating, and admitting appropriate patients into the Hospital at Home program. Unlike traditional hospitalist APP positions, this role is heavily centered on patient selection, interdisciplinary collaboration, care coordination, and program growth.
This is an opportunity for a clinically strong APP who enjoys autonomy, collaboration, and helping build a growing care model.

Ideal Candidate Profile
Required:

• 3-5+ years of Hospitalist APP experience
• Experience managing acute medical patients
• Strong inpatient medicine background
• Comfortable evaluating:
  • Acute Kidney Injury (AKI)
  • Infections
  • CHF
  • COPD
  • General Internal Medicine admissions
  • Complex acute care patients

Strong Communication Skills
Ability to:

• Build trust with patients and families
• Explain the Hospital at Home model
• Address concerns and answer questions
• Obtain patient buy-in and consent

Interdisciplinary Collaboration
Candidates must be comfortable:

• Partnering with hospitalists
• Communicating with specialists
• Coordinating with nursing, case management, pharmacy, PT/OT, and care coordinators
• Advocating for appropriate patients

Schedule & Coverage Needs
Standard Shift

• 7:00 AM - 5:00 PM
• 10-hour shifts

Coverage Requirements
Part-Time Candidates Must:

• Have meaningful weekend availability
• Be willing to work approximately every other weekend or similar rotation

Examples of schedules being considered:

• 6 shifts every 2 weeks
• 3 shifts every 2 weeks
• Weekend-focused schedules
• Custom arrangements for strong candidates

Weekend Rotation

• Approximately 4 weekends every 12 weeks

Compensation: $61.00 - $79.67/hour
