The Judge Group is hiring a Nurse Practitioner for a Senior Community Care (SCC) program in the Hanover, PA area . The NP will be part of a longitudinal, integrated care delivery program that coordinates the delivery and provision of clinical care of members in a facility setting.
Schedule
Mon - Fri, 8 am - 5 pm
no on-call, no weekends. no holidays required
Responsibilities:
- conduct comprehensive assessments
- effectively manage acute and chronic medical and behavioral conditions
- ensure accurate and complete documentation, including ICD 10 conditions
- conduct advanced illness and advanced care planning conversations
- provide patients and caregivers with counseling and education
- active FNP, ANP, GNP, AGACNP Board Certified license
- active DEA licensure/prescriptive authority
- full COVID-19 vaccination
