POSITION PURPOSE AND SUMMARY

The Neuroscience Nurse Navigator is responsible for creating, developing, and managing the post-discharge navigation program for the Neuroscience service line. With a broad knowledge base in neuroscience disease prevention, diagnosis, and treatment, this role delivers comprehensive clinical care coordination across the care continuum — with a primary focus on the critical transition period between hospital discharge and outpatient follow-up. Working in collaboration with the multidisciplinary healthcare team and in partnership with a non-clinical Health Navigator, the Neuroscience Nurse Navigator provides navigation program coordination and oversight, clinical surveillance, patient, family, and caregiver education, and evidence-based resources to facilitate informed decision-making, medication adherence, reduction in readmission, and secondary prevention. This role coordinates disease-specific programs aligned with organizational and governing body objectives, guides the multidisciplinary team in standardized evidence-based care delivery, and maintains oversight of programmatic performance, regulatory requirements, and quality/data reporting.

EDUCATION/EXPERIENCE :

  • Licensed Registered Nurse licensed in Indiana.
  • BSN required; MSN preferred
  • Strong Neuroscience/Stroke knowledge.
  • Stroke Nurse certification preferred – SCRN or CNRN
  • Minimum two years’ experience caring for stroke/neuroscience patients
  • BLS & ACLS certification.
  • Excellent patient assessment skills including assessment of psychosocial issues.
  • Knowledge of self care resources and strategies
  • Critical thinking skills
  • Strong leadership skills
  • Basic knowledge of insurance reimbursement systems, financial hardships, and payer coverage
  • Working knowledge of national, regional and community resources
  • Able to work independently and collaboratively with members of the healthcare team.
  • Experience with EMR and Microsoft Word Products.
  • Resourceful and adaptable to the changing healthcare climate.
  • Valid Driver’s License and automobile insurance for travel to various sites.
