RN Nurse Navigator Full Time

USPI Arizona Specialty Hospital is searching for a Nurse Navigator to join our wonderful team in the Pre‑OP PACU Department.

USPI Arizona Specialty Hospital is a specialty inpatient hospital established to set a new standard for excellence and patient satisfaction in the delivery of orthopedic and spinal surgery. Arizona Specialty Hospital provides excellent patient services and the highest level of care, with the overall goal of improving the delivery of healthcare in Maricopa County and beyond.

This is a Full Time Position; Monday‑Friday.

The Nurse Navigator will assume responsibility and accountability for the entire care pathway of the orthopedic and/or spine patient. Optimization of the program will be achieved by serving as a liaison throughout the continuum of care. In addition to the day‑to‑day activities required, the position requires the Navigator to participate in data collection, analysis, and reporting of clinical outcomes and patient satisfaction.

Position Responsibilities

  1. Works closely with the multidisciplinary team to develop and implement the patient’s care plan through assessment of the patient’s physical, educational, and psychosocial needs.
  2. Serves as an essential link between patients and all other healthcare providers.
  3. Serve as a resource to the patient through their continuum of care, including prior to surgery extending after discharge.
  4. Contacts patients to schedule preadmission testing, as ordered by the physician. This includes lab work, chest x‑ray, EKG, CT, and hospitalist medical clearance appointments.
  5. Ensures PAT results are relayed to the ordering physician.
  6. Contacts patients prior to surgery, to obtain health history and medication history.
  7. Reviews records for: appropriate orders, History and Physical, applicable clearances, labs, x‑ray, and EKG to assess patient’s health status prior to surgery.
  8. The nurse will use her/his nursing judgment (including the tools provided) to refer surgical patients, based on their provided medical history, to a hospitalist for review, if the patient is deemed a high‑risk surgical patient.
  9. During the prescreening phone call, the RN will provide preoperative instructions to the patient.
  10. Facilitates scheduling of the pre‑operative education class for the patient and care partner.
  11. Conducts patient rounds on assigned patients at least once during their hospital stay.
  12. Facilitates scheduled follow‑up calls with patients to promote compliance with discharge instructions and risk mitigation calls to avoid preventable ER visits/admissions, confirm physical therapy and post‑op visits to physicians.
  13. Models the Expected Behaviors for Patient Centered Care and is passionate about the patient experience.
  14. Ensures all necessary follow‑up/action items are completed, reported and documented in order to close the loop.
  15. Able to work as a team member, and encourage and promote a culture of caring.
  16. Participates in QAPI actives, PROM reporting, and completing EDGE requirements.

Position Qualifications

  • Hold and maintain a current Arizona license to practice as a Registered Nurse.
  • BSN preferred.
  • BLS (required).
  • ACLS (Preferred).
  • Minimum 3‑5 years of experience in a hospital setting, participating in the care of surgical patients. Orthopedic and spine experience is preferred.
  • Must be passionate about patient education.
  • Requires the clinical expertise to apply sound judgment to ensure optimal patient care.
  • Strong communication skills are required to instruct patients, family members and staff.
  • Must be self‑directed with strong interpersonal, organizational, and computer literacy skills.
  • Accepts and supports changes in work assignment as related to schedule and patient needs.
  • Ability to participate in data collection, analysis, and reporting.
  • Knowledge of relevant equipment, systems, change theory, policies, procedures, and strategies to promote effective quality outcomes.
  • Ability to utilize/apply the general and specialized principles, practices, techniques and methods of utilization review/management, care coordination, transfer coordination, discharge planning or case management.
  • Demonstrates the ability to work as a team member and to communicate directly and professionally. Is flexible, reliable, productive, patient‑oriented, and self‑motivated.
  • Maintains professional conduct and appearance.
